COLLECTION OF PERSONAL INFORMATION
Personal Information is information about you that identifies you as an individual, for example, your first and last name, address, e-mail address, or telephone number.
We collect information that you voluntarily provide to us through responses to surveys, search functions, questionnaires, feedback, payment forms and the like. On some of our Sites, we offer health assessment tools that ask you to provide self-assessment information. We may also ask you to provide additional information such as your e-mail address if you want to obtain additional services, information, participate in a contest or to resolve complaints or concerns.
HOW DOES EITZ CHAIM USE INFORMATION GATHERED ABOUT ONLINE VISITORS?

With the permission of an online visitor, information submitted at the time of submission may be used for marketing and promotional purposes by Eitz Chaim provided notice of this fact is made available online. Eitz Chaim uses reasonable efforts to maintain visitors’ information in a secure environment. Eitz Chaim, in common with many web site operators, may use standard technology called “cookies” on its Sites. You can disable cookies by turning them off in your browser; however, some areas of the Sites may not function properly if you do so.
If you visit our login page, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.
When you log in, we will also set up several cookies to save your login information and your screen display choices. Login cookies last for two days, and screen options cookies last for a year. If you select “Remember Me”, your login will persist for two weeks. If you log out of your account, the login cookies will be removed.

Pages on this site may include embedded content (e.g. videos, images, articles, etc.). Embedded content from other websites behaves in the exact same way as if the visitor has visited the other website.

We have put in place physical, electronic, and managerial procedures to safeguard and help prevent unauthorized access, maintain data security, and correctly use the information we collect online. Eitz Chaim applies security safeguards appropriate to the sensitivity of the information, such as retaining information in secure facilities and making personal information accessible only to authorized employees on a need-to-know basis.

If you have an account on any of our Sites, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us.
